I believe it's every time, since you can only battle most trainers once. I think it's later additions they enable rematches and you still get paid! It's Battle-city where you get points instead of money. :3

I think after Pokémon Emerald/Ruby/Sapphire they didn't have it, I don't remember ever seeing it :/

Umm... I don't remember much about Saphire cuz that's the last Pokemon game I played and I was already out of school so I didn't play it as much as the previous Pokemon games =/
